Food for Life
|
The goal of the Food for Life Program is to meet the basic nutritional needs for eligible, currently-enrolled SAAF clients. If eligible, clients work with their case manager and physician to determine the goal and the length of time the client will be on the program. The average client in the program meets their goal of weight gain, strengthening and independence within 6 months.
The Food for Life Program is intended for homebound clients who face the challenge of preparing their meals due to post-hospitalization, chronic and acute illness.
Dining Out Program
Many clients in the Food for Life Program have physical and financial limitations and going out to a restaurant is generally not an option. Chefs at five local restaurants (below) prepare and donate a special meal each month which is delivered to the clients' home by volunteers.

| Food Pantry |
The Food Pantry assists low-income clients and their family members to meet their nutritional needs by providing food to supplement their food budget.
The Food Pantry works in partnership with the Community Food Bank (CFB). While SAAF supplements food bags with items such as frozen meats and cheese, the basic food bag follows the Community Food Bank guidelines.
- Client must be eligible and current at SAAF.
- A client may receive up to two food bags every 30 days.
- Clients are able to pick up a supplemental food bag every 30 days. Supplies vary each month.
- Harvest Days are held 2 days of each month and include fresh fruits and vegetables.

| Medication Assistance Program (MAP) |
The Medication Assistance Program (MAP) provides help to clients in need of HIV/AIDS related medications. Clients must be eligible and active and all community resources have been exhausted to receive MAP.
The Medications Assistance Program provides short-term medication vouchers (up to 3 times) for people who have no other means of obtaining the prescription and who have a plan for prescription coverage in the future. (An annual maximum benefit applies).
MAP also provides assistance with medication co-payments for medications on the ADAP Formulary. (An annual maximum benefit applies).
SAAF provides assistance to eligible clients with COBRA and HMO insurance premium payments. (An annual maximum benefit applies).

| Complementary Therapies |
The Complementary Therapies Program provides access to a Naturopathic physician and therapies including acupuncture, massage therapy, chiropractic care, personal training, sweat lodge and nutritional supplements (purchased through the Travis Wright Memorial Buyers’ Club) for clients with limited incomes.
The Complementary Therapies Program is open to registered clients of SAAF who have been referred by their primary care physician, with a CD4 count of 500 or less and who meet income eligibility guidelines.
Program enrollment begins April 1. In the current year, 65 clients are enrolled in the program, which is the program’s capacity. The Program maintains a waiting list. Contact your case manager or peer counselor if interested in applying.

| Wellness Program |
The Wellness Clinic is coordinated and staffed by volunteers. The Wellness Clinics provide acupuncture, massage, and chiropractic care to eligible registered SAAF clients who are not eligible for the Complementary Therapies Program.

| Dental Services |
Dental services at SAAF are tied to the Statewide Title II Dental Services Program. Through this program, clients are enrolled in Delta Dental of Arizona and have an annual benefit of $1,500.00 (calendar year). This program covers Routine and Basic services at 100% and Major services (bridges, dentures, etc.) at 50%.
Eligibility includes: currently-enrolled in Case Management, income can not exceed 300% of the Federal Poverty Guideline and the client cannot have other dental coverage. For more information about the Title II Dental Services Program, please contact a Case Manager or Peer Counselor.
Additional assistance with dental services above the limit of the Title II Dental Services Program may be available through the Dental Supplemental Program. Please contact a SAAF Case Manager or Peer Counselor for additional information.

| Optical Services |
Current eligible clients of SAAF can receive eyeglasses no more than every 18 months as a general guideline.

| Hygiene and Cleaning Items |
SAAF assists low-income clients with hygiene and cleaning items.
- Client must be eligible and current at SAAF.
- Clients may receive 3 hygiene items and 2 cleaning items every 30 days.
- Hygiene supplies vary but usually include soap, toothbrush, toothpaste, shampoo, toilet paper, personal hygiene items.
- Cleaning supplies vary but usually include laundry soap, dish soap, cleaner and bleach.

| Transportation |
SAAF assists clients who are low-income and currently enrolled with SAAF with subsidizing bus passes.
Bus passes are distributed once a month.

| Holiday Project |
The Holiday Project is a collaborative project of SAAF and TIHAN. The goal of the project is to assist clients during the holiday season in providing support, gifts and food.
The gift-giving component of the project focuses on providing gifts to children and parents who are infected or affected.
Holiday meals, when available through donations, are provided to families or client groups.
All services provided through the Holiday Project are dependent upon community donations received.

| Attendant Care |
SAAF provides home health care services to eligible clients to assist them in remaining safely and independently in their home.
The program is provided to registered clients in need of home health care services who are low income and do not have other resources to attain these services. A referral from the physician is required.
The program provides limited home health care services following hospitalizations as well as limited on-going services for clients with chronic medical needs.
